What is your Health Profile?

This leaflet will help you understand how to fill in your Health Profile.

Everyone is different, so write about your health and the help and support that you need.

You can use the Health Profile instead of the hospital passport.  It is better to use the Health Profile because it includes all the information your health workers need. It can be used in visits with all the different types of health workers as well as in hospital.

Important things to remember:
  • Someone can help you fill in your Health Profile if you want.
  • Check all your information every year. If something changes, fill in a new Health Profile.
  • For example, if your medicine or tablets change, fill in a new Health Profile, change the medicine and change the date you filled in the new form.
  • Write clearly and give clear information, so it is easier for other people to use the information.
  • Take your Health Profile to all your health appointments.

    Wales Learning Disability Helpline

    The Wales Learning Disability Helpline is available to everyone in Wales, whether you are a person with a learning a learning disability, a family member or a friend.

    We can give you the information you need to understand the rights of people with a learning disability and to support them to access services or challenge decisions.

    Advocacy Matters Wales

    Provides services for adults with a learning disability. They also provide advocacy services for parents with learning disabilities.

    Learning Disabilities Liaison Service

    The Learning Disabilities Liaison Services Team offers support to adults who have a diagnosis of learning disabilities who are attending the hospital for outpatient, day patient and inpatient care. The team will work alongside community learning disabilities teams, care providers, families and carers to achieve the best health outcomes for the people they support

    Cardiff Council - Social Services for Adults 

    If you have (or someone you know has) a learning difficulty and think you may need help we can arrange for a social worker to carry out an assessment to see if there is something we can do.

    To get help you must be 18 years old or older.

    Request an assessment Once you request an assessment a social worker will contact you to find out some information about you and then work out what services we might be able to offer you.
