COVID Vaccination Forms - Request a vaccination online
Cardiff & Vale University Health Board have now created two online forms, which are available for patients to register their details if they have not yet had the vaccination.
There is currently a Vaccination Standy Form, which allows you to register your details and be contacted for a vaccine on short notice, as well as a 'Leave Nobody Behind' Form where you can register your details if you feel you have been missed from the current priority groups being vaccinated.
