Pregnancy Advisory Service (PAS) _{are trialling a new referral form in the Cardiff & Vale area. If you have an unplanned pregnancy and are considering abortion treatment, please fill in this form.}

•  Your name and address and date of birth
• A contact telephone number
• The date of the first day of your last menstrual period
• Your GP’s or the surgery’s name and address

 The booking clerk will ask for permission to call your mobile in case of clinic changes and similar issues.

The lines are open Monday-Thursday 8:30am-12:30pm and 1:30pm-4:30pm and Friday 8:30am-12:30pm and 1:30pm-4:00pm.
